UNIVERSITY CHALLENGE TEAM RELAY SWIMMER APPLICATION

Enrollments are considered on a first-come, first-served basis. Please remember, applications are not considered to be complete until all the required information has been received and approved by the SWIM Committee. The SWIM Committee will review your application and will notify you via E-mail of your acceptance status within two weeks. Preference will be given to those individuals and teams who have submitted all of their qualification information and medical documentation at the time of enrollment.

To apply you are required to do the following:

First you must have received an E-mail from your Team Captain with a link to complete your registration. Please follow your Team Captain's E-mail link to complete your registration. Only after registration is completed, may you continue to the steps below.

  • Complete the the application form
  • Then, send the completed PDF application form to us either by:

    NOTE: Photos should be no larger than 3MB in size, preferably 640x480 pixels or larger
